DISCLAIMER

MissionMed Wellness is strictly a telehealth platform. This website does not provide medical advice. Video conferencing technology is not the same as a direct patient/health care provider visit and has limitations in regard to the physical examination. Telehealth consultation has potential benefits, including easier access to care, decreasing costs, and allowing visits to be performed from the comfort of my home. It also has potential risks including interruptions, unauthorized access, and technical difficulties.

MissionMed Wellness is not an Emergency Service. In the event of an emergency or urgent medical issue please call 911 or visit the nearest emergency department or urgent care facility.

MissionMed Wellness does not sell any medical/health products or work with any vendors at this time.

COMMUNICATION

The preferred method of communication is by our contact forms, HIPPA-compliant Patient Portal, text and email. Transmitting confidential health information by e-mail or text, however, has a number of risks: E-mail can be immediately broadcast worldwide and be received by many intended and unintended recipients; recipients can forward e-mail messages to other recipients without the original sender(s) permission or knowledge; users can easily copy information. The practitioner / practice will endeavor to read e-mail promptly. However, MissionMed Wellness can provide no assurance that the e-mail will be read immediately. Therefore, e-mail must never be used in a medical emergency. Phone calls are generally reserved for Direct Primary Care (DPC) members or exceptional circumstances, as the provider is often with patients.

PRIVACY POLICY

OUR LEGAL RESPONSIBILITIES:
MissionMed Wellness agrees to maintain the privacy of protected health information and provide you with notice of our legal duties and privacy practices with respect to your protected health information.

HOW WE MAY USE OR DISCLOSE YOUR PROTECTED HEALTH INFORMATION:
MissionMed Wellness may use and disclose your protected health information to provide you with treatment . This includes disclosing your protected health information to other medical providers, trainees, therapists, medical staff, and office staff that are involved in your health care. In addition, we will need to utilize your information to contact you be telephone, email, or text . It is the policy of MissionMed Wellness that all email messages related to a patient’s diagnosis or treatment will be considered part of the patient’s protected health information. However, MissionMed Wellness cannot guarantee the privacy of electronic communications and is not liable for unauthorized disclosure of confidential information due to patient negligence.

INSURANCE FREE POLICY

To ensure you receive the exceptional care you deserve without insurance limitations, MissionMed Wellness does not accept insurance. Patients receiving care from our practice are not permitted to submit bills paid to us for reimbursement by their insurance company. 

FINANCIAL POLICY

MissionMed Wellness does NOT take insurance, neither substitute insurance coverage for catastrophic events. Every client is responsible to pay for service at the time of booking and before the end of that service if labs or additional tests are ordered. Client is responsible and agrees to pay the laboratory fees and MissionMed Wellness diagnostic analysis fee, prior to having the lab drawn. You may pay by, credit card, debit card or Zelle. We will provide you with a superbill per request. Insurance reimbursements are NOT permitted neither honored for any services with us.

Membership fees are billed on a monthly basis and can be canceled at any time. However, members are required to provide at least 7 days notice before the start of the next billing cycle to cancel their membership. We have the right to change these policies at any time.

MISCONDUCT

MissionMed Wellness is committed to treat every individual with respect and dignity. We strive for a pleasant, respectful and trustworthy relationship with our clients.

In the event of misconduct, including foul language, inappropriate remarks and behavior, harassment, hostility towards the provider, as well as abuse of the membership, MissionMed Wellness reserves the right to terminate membership or deny further subscriptions or services.

MISSED & CANCELLATION APPOINTMENTS

MissionMed Wellness requires 48-hour notice if you are unable to keep your new patient or initial visit and 24-hour notice if you cannot keep your follow-up or routine visit. If you miss an appointment or fail to give sufficient notice, you will be charged $30.00 for that missed appointment. This payment will be deducted from the payment made at the time of booking the appointment.

PAYMENTS & PAST DUE ACCOUNTS

For non-members, a full payment is required at the time of booking of any service. 

For DPC members, if the credit card is declined or 2 recurrent payments are missed within the membership plan, your account will be referred to our collection agency and MissionMed Wellness will automatically cancel your membership. If you wish to resubscribe for membership, you will be charged a $70 enrollment fee in addition to your current account balance. 

For members that subscribe and cancel membership on 2 or more occasions, the enrollment fee will be $100; however, MissionMed Wellness reserves the right to deny a renewed partnership in such cases.

REFUND POLICY

Refunds are not provided for completed acute visits or for visits by non-members where medical advice and/or prescriptions were given. MissionMed Wellness does not offer refunds for any weight loss products ordered, whether used or unused. Additionally, we do not offer exchanges for services. For members, there are no refunds if the service has not been utilized and its benefits enjoyed. Members can cancel the membership at any time. Please see financial policy.
